<br />support to resolve numerous adequacy issues identified in earlier <br />reviews. Their efforts were unsuccessful, so on January 17, 1989, <br />NCEC submitted to the Division a letter formally requesting <br />renewal of the permit allowing only reclamation. A reclamation <br />only permit was issued in September, 1989. <br />Six months later, on March 9, 1990, NCEC submitted to the Division <br />an application for Permit Revision No. 1 to reactivate the mining <br />permit status and extend the permit term from three to five years. <br />Permit Revision No. 1 was approved with stipulations by the Mined <br />Land Reclamation Board on December 18, 1990. Permit Revision No. <br />2, submitted by NCEC on February 26, 1991, added a loadout facility <br />and wad approved on September 4, 1991. <br />In April, 1991, NCEC submitted Permit Renewal Application No. 2. <br />In May, 1991, Coal Ridge No. 1 underwent a succession of operators <br />from NCEC to NCIG Financial, Inc. Because of issues raised in the <br />adequacy review for the permit renewal application, the .decision <br />date for the renewal was extended, and permit renewal was finally <br />issued on November 20, 1992. <br />On November 5, 1993, the Garfield County Special Use Permit for the <br />Coal Ridge No. l Mine expired. With the expiration of the County <br />Special Use Permit, the reclamation schedule for the mine was <br />activated. <br />The midterm date for this five-year permit term is April 10, 1994. <br />This Midterm Review was initiated in January, 1994. <br />Since permit renewal, one Notice of Violation has been issued. <br />NOV C-92-009 was issued for failure to maintain mine ID signs and <br />perimeter markers at the loadout facility. The NOV was terminated <br />on August 17, 1992, and a notice and order to pay a civil penalty <br />was issued by the Division on October 9, 1992. Payment was <br />received on December 11, 1992. <br />SECTION II - REVI8I0N8 TO THE PERMIT <br />This section summarizes revisions to the Coal Ridge No. 1 Coal <br />Mining Permit during the current permit term which began with the <br />expiration of the last permit term in October, 1991. <br />Technical Revisions - No technical revisions have been issued <br />during the current permit term. <br />Minor Revisions - <br />Minor Revision MR-10 was approved on March 30, 1992 for the <br />replacement of Monitoring Well OW-4L. <br />Minor Revision MR-11 was approved on September 18, 1992 for the <br />certification of maps and minor corrections to the permit <br />application package. <br />2 <br />
