On Site Processing: Solution (SO) Chemical (CH) Thermal (TH) <br />Heap Leach (HL) Vat Leach (VL) <br />None -All ore will be processed otl'-site <br />Description of Amendment of Conversion: If you are amending or converting an existing operation, provide a brief <br />narrative describing the proposed change(s): The current mining operations, permitted under a 110 permit, will be <br />expanded from 9.9 acres of affected lands to 17.8 acres of proposed disturbance and an additional 12.2 acres of potential <br />future disturbed acreage for a total of 30.0 acres of affected lands. This expansion will increase the affected lands over the 10 <br />acre limit, requiring this permit to be converted [o a 112 permit. <br />Maps and Exhibits: <br />Two (2) complete, unbound application packages must be submitted. One complete application package consists of <br />the signed original application form and the se[ of maps and exhibits referenced below as Exhibits A-S and the <br />Geotechnical Stability Exhibit. Each exhibit within the application must be presented as a separate section. Begin <br />each exhibit on a new page. Pages should be numbered consecutively for ease of reference.
