Preliminary review of the information received determined that the following items must be received before the Division can consider your application as being submitted and technical review can begin: APPT.iCATTnN FnRM~ 'lye Division did not receive a completed application form. Description shall be by township, range, and sectioq to at least the nearest quarter-quarter section. A metes and bounds survey description is also acceptable. Also, the street address or lot number(s) may be used. This information is available from the County Assessor's office or U.S. Geological Survey (USGS) maps. I~~TRTT F.-= Mnn ,(RrdP 63.51- (l) In addition to the requirements of 6.2.1(2), you must provide a map that clearly describes the features associated with the mining plan and the components of the Reclamation Plan. Include one map for fire mine plan and one map for the Reclamation Plan. The map(s) must be drawn to a scale no smaller than appropriate to cleazly show all elements that are required to be delineated by the Act and these Rules; show a north arrow, note any section corners adjacent to the proposed operatioq and indicate the date illustrated. At a minimum, maps must include the following information: (2) Mining Plan Ma~n_, (a) outline and label the pern3it boundaries, described in Exhibit A -Legal Description; for all Limited Impact 110 and Special 111 Operations, the Office considers the azea bounded by the pemut boundary to be analogous to the affected azea; (b) label the names of owner(s) of record of the surface of the affected azea, of the land within two hundred (200) feet of the affected area, identify the owner of the substance to be mined, and the type of structure and owners of record of any permanerFt or man-made structures within 200 feet of the affected area; Office of STATE OF COLORADO Office of Colorado
