Preliminary review of the information <br />received determined that the following items must be received before the Division can consider your application as being <br />submitted and technical review can begin: <br />APPT.iCATTnN FnRM~ 'lye Division did not receive a completed application form. Description shall be by township, range, and <br />sectioq to at least the nearest quarter-quarter section. A metes and bounds survey description is also <br />acceptable. Also, the street address or lot number(s) may be used. This information is available from the <br />County Assessor's office or U.S. Geological Survey (USGS) maps. <br />I~~TRTT F.-= Mnn ,(RrdP 63.51- (l) In addition to the requirements of 6.2.1(2), you must provide a map that <br />clearly describes the features associated with the mining plan and the components of the Reclamation Plan. <br />Include one map for fire mine plan and one map for the Reclamation Plan. The map(s) must be drawn to a scale <br />no smaller than appropriate to cleazly show all elements that are required to be delineated by the Act and these <br />Rules; show a north arrow, note any section corners adjacent to the proposed operatioq and indicate the date <br />illustrated. At a minimum, maps must include the following information: <br />(2) Mining Plan Ma~n_, <br />(a) outline and label the pern3it boundaries, described in Exhibit A -Legal Description; for all <br />Limited Impact 110 and Special 111 Operations, the Office considers the azea bounded by the pemut boundary <br />to be analogous to the affected azea; <br />(b) label the names of owner(s) of record of the surface of the affected azea, of the land within two <br />hundred (200) feet of the affected area, identify the owner of the substance to be mined, and the type of structure <br />and owners of record of any permanerFt or man-made structures within 200 feet of the affected area; <br />Office of <br />STATE OF COLORADO <br />Office of <br />Colorado <br />
