-G- <br />Permit Conditimt <br />A reportable spill is a spill of any toxic or hazardous sutsstance (including spills of pevoieum products) witMn cite <br />mined land permit area reportable to any Division of the Colorado Department of Public Heatth and the Environment, the <br />National Response Center, the Colorado Emergency Plannirg Commission, any local Emergency Planning Commission, <br />local Emergency Planning Committee, or the State Oil Inspector. The Mine Operator shall notify the Division of <br />Reclamation, Mining and Safety (DBMS) of a reportable spill within the mined land permit area using the same timefiame <br />required by the permit, license, notice, statute, rule or regulation governing the reporting of the spill to the other appropriate <br />agency. Notice ofa reportable spill shall be FAXed to: Minerals Program Supervisor, Divisionof Reclamation, Mining and <br />Safety, FAX (303) 832-8106. The FAX shalt include a call-back number ofa responsible company of£cial for DBMS staff <br />to use as a contact. <br />NOTE'1'O COMh1ENTORS/OBJECTORS: <br />It is likely there will be additions, changes, and deletions to this document prior to final decision by the Office. 'fherotbre, if you have <br />any comments or concerns you must contact the applicant or the Office prior to the decision date so that you will know what changes <br />may have been made to the application document. <br />The Office is not allowed to consider comments, unless they are written, and received prior to the end of the public comment period. <br />You should contact [he applicant for the final date of the public comment period. <br />If you have questions about the Mined Land Reclamation Board and Oftce review and decision or appealsprocess, you may contact the <br />O[4ice at (303) 86G-3567. <br />
