04038 Banks and Gesso. LLC <br />Exhibit M -Other Permits and Licenses <br />County Land Use Permit <br />The applicant is applying for a Routt County Use by Special Review to allow mining on <br />the site as well as a concrete and asphalt plant. <br />County Flood Hazard Area Development Permit <br />A Flood Hazard Development Permit will not be needed since there are no flood hazard <br />areas within the proposed permit boundary. <br />CDPS State Discharge PermiUStormwater Permit <br />A CDPS State Discharge Permit will not be needed due to lack of discharge into streams <br />from the operations at this site. All stormwater will drain into mined areas and either <br />seep into the ground or evaporate. <br />Air Permits <br />The applicant will maintain, transfer and/or obtain all necessary air permits (for fugitive <br />dust and processing plants) from the State prior to the start of mining operations. <br />Road Access and Maintenance Aoreements <br />A CDOT Access Permit will be obtained, if necessary, for access onto State Highway <br />131. <br />Substitute Water Suggly Plan/Gravel Pit Well Permit <br />A Substitute Water Supply Plan or Gravel Well Permit will not be needed since there will <br />be no exposure of groundwater. <br />State Historic Preservation Office <br />Please see the results of a search of the Colorado Inventory of Cultural Resources in <br />Attachment C. There are no historic sites present within the proposed permit boundary. <br />U.S. Armv Corgs of Engineers 404 Permit <br />A 404 Permit will not be needed since there will be no disturbance of wetland areas. <br />King Mountain Sand & Gravel Mine 04038 <br />King Mountain Gravel, LLC 21 November 2004 <br />
