Cattany <br />Division Director <br />Natural Resource Trustee <br />Re: Nickell Gravel Pit, Permit No. M-1994-077, Receipt of 11 0 112 Construction Materials Reclamation Permit <br />Conversion Application Package, CN-1 L~ <br />Deaz Mr. Charr>p: <br />On May 31, 2006, the Division of Minerals and Geology received your 110 to 112 Conshuction Materials Reclamation Permit <br />Conversion Application package for the Nickell Gravel Pit, Permit No. M-1994-077. Preliminary review of [he information received <br />determined that the following items must be received before the Division can consider your application as being submitted and <br />technical review can begin: <br />APPi.irATTON FORM: page 2, #10: Need Longitude; Page 4, #16: Need description of Conversion. <br />F.XHIRIT A -legal Description [Ante 6.4.11_ The legal description must identify the affected land and be wholly adequate <br />for field location of the property. It can be in the form of metes and bounds survey or a description by reference to township, <br />range and section to at least the neazest quarter quarter section. Where applicable, the street address or lot numbers may be <br />used. <br />EXFtiRIT R - Prnnf of Filing with Cnunly ('lerk and Recorder (Role 6.4.1 R1: You must submit an affidavit or Ieceipt <br />indicating the date on which the application was placed with the local County Clerkand Recorder. <br />Your application will not be considered submitted until the information listed above is received and found sufficient to begin olu <br />review. A decision date will be established 90 days from the date of receipt of all of the requested information. Additionally, if you <br />have already published notice you will need to republish notice, after the Division considers the applicafion submitted. This notice <br />must be published once a week for fotEr (4) consecutive weeks, starting within ten (]0) days of the date your application is considered <br />submitted. We will notify you when you should initiate republication of your notce. The final date for receiving cormnents is the 20th <br />day after the fourth publication or the next regulaz business day. <br />You have sixty (60) days from the above date to submit all necessary documents that the Office needs for an application to be <br />considered filed. If, at the end of the sixty day period, the application has not been determined to be filed with the Office, the Office <br />shall deny the application and [emrinate the application file. - <br />This letter shall not be construed to mean that there aze no other technical deficiencies in your application.
