The Division of Reclamation, Mining and Safety (Division) is required to make an approval <br />or denial decision no later than October 5, 2006, therefore, your response to the following adequacy review <br />concerns should be submitted to the Division as soon as possible. <br />Rule 1.6.2(11(gl -General Apnlicant Procedures <br />In accordance with this rule, please submit proof of the public notices submitted to al] owners of record of <br />surface lands, easement holders, and owners of record of surface lands that aze within 200 feet of the <br />boundary of affected land. Proof of Notice may be satisfied by submitting return receipts of a Certified <br />Mailing or by proof of personal service. <br />Rule 6.2.1(2 (b) 1-Maps and Exhibits <br />In accordance with this rule, maps, with the exception of the index map, must be prepared and signed by a <br />registered land surveyor, professional engineer, or other qualified person. Please resubmit signed copies of <br />Exhibit C-4, Mining Plan Map, and Exhibit F, Reclamation Plan Map. <br />Rule 6.4.3 Exhibit C -Pre-Mining and Mining Plan Map(sl of Affected Lands <br />3. In accordance with Rule 6.4.3(d), please modify Exhibit C-4, Mining Plan, to include the silt storage area. <br />Rule 6.4.4 Exhibit D - Minin>? Plan <br />4. In Exhibit D, Mining Plan, the applicant states that the berm which separated the Seaworth Parcel from the <br />Taft Hill Expansion Pit was mined through but subsequently reconstructed.
