THIS LAMINATED NOTICE IS POSTED AT ALL COHIAIt~NT STRUCTURES /Q <br />./(/N cFiV~c~ <br />REPORTABLE SPILLS PROCEDURE''°°.,,yn ~~~003 <br />°`a/s <br />agd~e%9y <br />A reportable spill is a spill of any toxic or hazardous substance <br />including petroleum products. <br />Notice of a reportable spill shall be faxed immediately to the <br />Minerals Program Supervisor, Division of Minerals and Geology <br />FAX 1-303-832-8106. <br />Which shall include a call back number of a responsible company <br />official. <br />A report shall also be made to Colorado Department of Public <br />Health & the Environment at <br />1-877-518-5608 <br />The report MUST include the following information: <br />• Operation Name: Western Slope Aggregates, Inc. <br />• Permit Number: DMG # M1981-207 MSHA# OS-04481 <br />• Name of person reporting <br />• Date and Time of spill <br />• Type of material spilled CAS # form MSDS <br />• Amount spilled, lateral extent of spill, whether any <br />material has left the permit area, and where the spilled <br />material migrated to. <br />• What measures are being taken to contain and clean up <br />the spill? <br />• Contact name of local State Health Department. <br />
