LOng <br />Division Director <br />On April 15, 2002, the Division of Minerals and Geology received your response to the Division issuing a new <br />permit for the site currerrtly issued in the name of Westem Slope Flagstone, Permit No. M-1996-076. The Division <br />has not processed your request due to the fact that yow letter requested that the Division put the permit in your <br />name and the Company Name as Westem Slope Flagstone and change the site name to Cameo Gravel Pit. The <br />Division cannot put the permit in both your name and Westem Slope Flagstone. It must be either put in yow name <br />or Westem Slope Flagstone. Currently the permit is issued to Westem Slope Flagstone and the site name is <br />Westem Slope Flagstone Quarry No. 2. The Division reviewed the file and we feel that the permit should have <br />been issued to "Rudolph Forrtanari" with the site name being Westem Slope Flagstone Quarry No. 2. The Division <br />feels that the permit should be issued to Rudolph Fontanari but we cannot issue a new pemut without your consent. <br />Please let the Division know if you would like the pemut reissued in your name. The site name can be changed <br />during the Conversion process with another letter from you requesting that the site name be changed to Cameo <br />Gravel Pit.
