Cottony <br />Acling Division Director <br />On Augast 15, 2002, the Division of Minerals and Geology received your Succession of Operators (SO) application, S 125.00 SO <br />fee, Performance Warranty, Financial Warranty Letter of Credit No. 428, and The Home State Back Standby Letter of Credit No. <br />428. The Division is returning the SO application, the Performance Warranty and the Financial Waaanty forms to you to have the <br />following changes made before the Division can process the transfer Permit No. M-1994-096 from James D. and Beckie J. Krakel <br />to either you or your Limited Liability Corporation. The Standby Letter of Credit is being returned to The Hame State Bank directly. <br />A copy of the letter sent with the Letter of Credit is enclosed far your information. <br />1. All Docamenta: Informatron contained in all documents must be consistent and accurate. Your Company <br />Name, Operation Name (Russell Gtavel Pit), and Permit Number (M-1994-096) must be consistent throughout <br />ALL documents. High Plains Sand and Gravel, LLC is not listed as a Trade Name in the Secretary of States web <br />page, it is shown as a Limited Liability Corpomtion and the word "and" is spelled out not the "&" symbol If you <br />do not want the Russell Grave] Pit trallsfersed to your company you will need to delete your company same from <br />all of the documents and use your name. Or, ifyon world h7re the permit transferred to Robert C and Helen L <br />Pittington then all of the documents must show only that name as the Operator. <br />2. Notarzation of Operator's Aclmowledaeme~/Notarv for Successor.
