(Page 2) <br />MINE ID # OR PROSPECTING ID #: ti+-tool-C137 <br />INSPECTION DATE: 5_~_ne INSPECTOR=S INITIALS: JiL <br />OBSERVATIONS <br />Completed an inspection of this site pursuant to provisions of Rule 4.16.2(2) offer the Division received a request <br />for full release of reclamation responsibility. <br />This is a small 8 acre 110 Limited Impact permN located about'/. of a mile north of Fairplayjust west of Highway <br />9. <br />Most of the site has been reclaimed per the approved permit; pit floor leveled (slightly sloping eastward), reject <br />material pushed over the east side of the permit area (cobble sized material) to forma 1:1 outslope. North end <br />was benched to create a stable slope. West side is still near vertical; as it was before mining initiated under this <br />permit. Site is stable. <br />Two noted problems: 1. A fuel tank is still within the permit area and 2. It appears the operator did not attempt to <br />establish the potentilla, mountain mahogany, rabbitbrush and sage shrub species as required by Stipulation #2 <br />of the approved permit. <br />The site is not ready for release. The operator must remove the fuel tank, level the containment facility and plant <br />50 shrubs (about 12 of each species) per acre (see attached permff Stipulation).
