Box 535, Castle Rock, C0 80104, an Sally Buckland, (hereinafter refen'ed td as the "Structure Owner") whose address is P.O Box 56. Empire: CO 80438, The Applicant is seeking an. Amendment to its State of Cobrado Mined Land Redamation Board Permit # M-77-534 to mine sand and gravel and to reclaim the property as identified in the legal description (see Attachment B) and as depicted on the enclosed Location.Map (see Attachment C). The Structure Owner owns a permanent man-made structure within 200 tt of the land affected by mining and reclamation operations. The description of :this structure(s) is given as Attachment A.
