Dear Mr. McCall:
On July 30, 2007, the Division of Reclamation, Mining and Safety received a Succession of Operators Application, a
$125.00 application fee, a Performance Warranty, and a Financial Warranty Check for Deposit in State Treasury, in the
amount of $9,481.00 for the Ruby Trust Mine, Permit No. M-1979-181. The Division is returning these documents for the
following:
Succession of Operators Application:
The Division requires current permitted operator's company name listed throughout this document match the
company name listed with the Colorado Secretary of State's office (i.e. "Walker Mining and Milling,
Incorporated" rather than "Walker Mining & Milling, Inc."). Please correct the company name listing
throughout this document and have your notary correct the listing and initial the correction within the Notary for
Pennittee section on page 5.
The Division requires completion of the Notary for Successor section on page 5. Please have your notary include
the title for Mr. Wiley McCall.
On July 1, 2007, the Succession of Operator Application fee was raised from $100.00 to $115.00. Please submit
the supplemental $15.00 of the application fee.
Performance Warranty:
The Division requres completion of the Notarization of Operator's Acknowledgement section on page 3. Please
have your notary include the title for Mr. Wiley McCall.
Financial Warranty Check for Deposit in State Treasury;
The Division requires completion of the Notarization of Operator's Acknowledgement section on page 4. Please
have your notary include the title For Mr. Wiley McCall.
Upon correction, please return these documents and the fee supplement to my attention.
