Dear Mr. Welte:
On September 5, 2003 the Division of Minerals and Geology increased the current Financial Warranty for this
permit to $21,170.00, in accordance with Rule 4.2.1 of the Rules and Regulations. This is an increase of
$11,070.00. A copy of the Cost Summary Form is enclosed for your review.
The Division ordered amendment of the current Financial Warranty, or submittal of a new Financial Warranty
reflecting the increase, shall be submitted to the Division within 60 days from the date of this letter. If you wish to
submit a different type of Financial Warranty, please contact me such that I may send you the applicable form.
