On October 8, 2002, the Division of Minerals and Geology received your 112 Cons4uction Materials Reclamation Permit
Amendment Application package for the Hogue/River Pit, Pemlit No. M-1992-066. Preliminary review of the information
received determined [hat the following items must be received before the Division can consider your application as being
submitted and technical review can begin:
APPLICATION FORM: Page 1, Item I: The Division requires that the Applicant/Operator name be the same as
the name on the permit (i.e Duckels Construction, Inc.). Page 1, Item 3: The Division requires the exact acreage.
Page 2, Item 9: Quarter Section and Quarter/Quarter Section need completing. Page 3, Correspondence
Information: The Division requires that the Applicant/Operator name be the same as the name on the permit. Page
7, Signatttre Page: The Division requires that [he Applicant/Operator name be the same as the name on the permit.

6.4.17: Please submit proof of actual delivery or proof of marling by Certified Mail, return receipt requested, of
Notice of the Application to the local Boazd of County Commissioners.
Your application will not be considered submitted until the information listed above is received and found sufficient to begin
our review. A decision date will be established 90 days from the date of receipt of all of the requested information.
Additionally, if you have akeady published notice you will need [o republish notice, after the Division considers [he
application submitted. This notice must be published once a week for four (4) consecutive weeks, staging within ten (lop
days of the date your application is considered submitted. We wdl notify you when you should initiate republication of your
notice. The final date for receiving comments is the 20th day after the fourth publication or the next regular business day.
This letter shall not be construed to mean that [here aze no other technical deficiencies in your application. The Division will
review your application to determine whether it is adequate [o meet the requirements of the Act after submittal of all required
items.
