Because this site is currently permitted by a County the Division cannot process your request to transfer the
permit to Reese Contracting until a Reclamation Bond calculation is done. Therefore, the following changes must be
made to your application:
1). Paee I: Please initial and date Option A and cross-through and initial Option B. Tlris allows the
Division time to do an inspection and bond calculation of the site to be transferred to Reese
Contracting. Option B does not apply in this Succession of Operators situation.
2). Page 2 -Item 5: You submitted a fee of $225.00. This is a $100.00 overpayment of the
Succession of Operators fee. The fee to transfer the permit to Reese Contracting is $125.00. The
Division has requested a refund of $100.00 to be sent to you under separate cover.
3). Pace 4 - Paraeraah 2: Please change the "new operator" name to "Reese Cornracting".
4}.
