Please be advised that this is the incorrect <br />application form to transfer the Roaring Fork Resources, Inc., Mamm Creek Sand & Gravel permit to Lafarge <br />West, Tnc. The Mamm Creek Sand & Gravel site was issued a permit on October 29, 2001, therefore, the correct <br />application to transfer the Mamm Creek Sand & Gravel site to Lafarge West, Inc. from Roaring Fork Resources, <br />Inc. is "Transfer of Permit and Succession of Operators Application Form" (form is enclosed for yow use). <br />The Division is returning the Financial Warranty Corporate Surety form, which you submitted on <br />December 9, 2003, because the date the Corporate Surety was executed and the date that the Power of Attorney <br />was executed do not match. The date that the Attorney-in-Fact signed and the date of the Power of Attorney <br />page must match. <br />Also, the Division did not receive a properly executed Performance Warranty form.
