The decision date for your application is scheduled for March 7, 2005. <br />Therefore, I would appreciate a response from you on or before February 25, 2005. (Please be sure to <br />include two {2) copies of your response). Please be advised that your conversion application may be <br />deemed inadequate and may be denied on March 7, 2005 unless the following adequacy review items are <br />addressed to [he satisfaction of the Division. <br />X6:4.2 E~CHIBIT B -Index Mau <br />1.
