Cattany <br />Division Direcor <br />Natural ReSOUrce Trustee <br />On January 11, 2005, the Division of Minerals and Geology received the 110 Consttnction Materials Reclamation Permit <br />Amendment Application package for Rudy Rudibaugh's Rudibaugh Pit, Permit No. M-1995-057. Preliminary review of the <br />information received determined that two (2) copies of the following items must be received before the Division can consider <br />your application as being submitted and technical review can begin: <br />APPT,iCATi(1N FORM: Please complete the highlighted aeeas of the application and resubmit the original <br />application and a copy of this application <br />F,XHiRTf A - ivtial Drscri~t;nn tRnle Fs-tall: The legal description must identify the affected land, specify <br />affected areas and be adequate to field locate the property. Description shall be by township, range, and section, to at <br />least the nearest quarter-quarter section. A metes and bounds survey description is also acceptable. Also, the street <br />address or lot number(s) may be used. This information is available from the County Assessor's office or U.S. <br />Geological Survey (USGS) maps. A second copy of this exhibit must be submitted. <br />F.XAiIiiT A - raraHnn Maw (Role fis_t12N: A map showing information sufficient to determine the location of the <br />affected land on the ground and existing and proposed roads or access routes to be used in connection with the mining <br />operation. Names of all immediately adjacent surface owners of record shall also be shown. The operation location map <br />shall be a standard 1:24,000 scale U.S. Geological Survey map. The location of the proposed operation shall be shown <br />and labeled with the mine site name. At a minimum, you must include a <br />description of the vegetation and soil characteristic$ in the area of the proposed operation. The local office of the <br />Natttral Resources Conservation Service (NRCS) may provide you with this information as well as recommendations for <br />Exhibit D -Reclamation Plan; <br />F.XAiRIT R _ Site 1)rcrrint;nn tRnlr 6.3.2rh1L• You must address Item (b) to the eaten[ necessary to demonstrate <br />compliance with the applicable performance standard requirements of Rule 3. At a minimum, you must identify any <br />permanent man-made structures within two hundred (200) feet of the affected area and the owner of each stmctare. <br />Permanent man-made structures include houses, barns, fences, above or below ground utilities, irrigation ditches, <br />maintained or public roads, bridges, wells, pipelines, water storage impoundments, railroad tracks, cemeteries, and <br />communication antennas, etc. Each structure should be located on Exhibit E -Map; <br />EXAiRiT R _ Gate 1)rsrri~ r;nn rRnle fi_3.2rr11: You must address Item (c) to the exKent necessary to demonstrate <br />compliance with the applicable perforntance standard requirements of Rule 3. At a minimum, you must give a <br />description of the water resources in the area of the proposed operation. Identify any sVeams, springs, lakes, stock water <br />ponds, ditches, reservoirs, and aquifers which would receive drainage directly from the affected area.
