Walther <br />Executive Director <br />Division Director <br />Re: Sumyside Gold Miue, Permit No. M-1977-378, Receipt of 112 Hard Rock/Metal'Mining Reclamation Permit <br />Amendment Application Package, AM - 5 ~ <br />Deaz Mr. Periao: <br />On April 9, 2003, the Division of Minerals and Geology received your 112 Hazd Rock/Metal Mining Reclamation Permit <br />Amendment Application package for the Sunnyside Gold Mine, Permit No. M-]977-378. Preliminary review of the infomtaflon <br />received determined that the following items must be received before the Division can consider your application as being submined <br />and technical review can begin: <br />APPi.i('ATT()N FnRM~ page 1, Item 2: The Division requires that [he operation name remain consisteht with what.is <br />currently on file (i.e. Sunnyside Gold Mine). Page 4, Responsibilities as a Permittee: The Division requires [hat these <br />items be initialed by a corporate officer. Page 5, Signature Page: The Division requires that this page be sign by a <br />corporate officer in the presence of a Notary Public. <br />FXiTlRTT T. - RarlamaHnn ('nett tRrde (..a_t2i (1) All information necessary to calculate the costs of reclamation <br />must be submitted and broken down into the various major phases of reclamation. A decision date will be established 90 days from the date of receipt of all of the requested information. Additionally, if <br />you have already published notice you will need ro republish notice, after the Division considers the application submitted. This <br />notice must be published once a week for four (4) consecufive weeks, starting within [en (10) days of [he date your application is <br />considered submitted. We will notify you when you should initiate repubhcation of your notice. The final date for receiving <br />comments is the 20th day after the fouRh publication or the next regular business day. <br />This letter shall not be construed to mean that there are no other technical deficiencies in your application.
