Adequacy Items
The following items must be addressed by the applicant in order to satisfy the
applicable requirements of C.R.S. 34-32-101 et seq. and the Mineral Rules and
Regulations of the Mined Land Reclamation Board:
1 . Application Form - please submit a new application form
(enclosed). his form replaces all our older forms. Please also
provide an additional $6.03 for your application fee.
2. Exhibit C -please provide the following information on your 1
excavation plan map: l

(a) all immediately adjacent surface owners of record;X
(b) the name and location of all significant, valuable andV/�
permanent man-made structures located within 200 feet of the
affected land. You will need to provide a 200 foot setback
from all such structures that you do not have a damage
compensation agreement for with the owners of these structures.
3. Exhibit D -please supply the following information for the proposed
amendment areas:
(a) a description of the methods of mining to be employed;
(b) a description of all earthmoving proposed; `
(c) a description of all water diversions and impoundments;✓ r
(d) a description of the disturbances as categorized in Rule�J / \
2.1 .2( 5) (d) ;
(e) a description of the approximate timetable for these proposed
areas as described in Rule 2.1 . 2(5) (e) ;
(f) a description of the nature, depth, and thickness of the ore
body or deposit to be mined and the thickness and type of
overburden to be removed;
(g) a description of the nature of the stratum immediately beneath
the material to be mined if in sedimentary deposits.
