All disturbance to be confined to 3,06, 0.745 acre <br />and 0.13 acre areas (already disturbed) as depicted on map received February 9, 1006. Operator stated no disturbance <br />in 1,43, 1.49 and 1.00-acre areas. <br />The terms of the Amendment No. 1 approved by the Division aze hereby incorporated into Permit No. M-1992-045.
