Upon permit issuance and prior to beginning on-site mining activity, you1°mu"at°p~~~~$`~~Y$~ffl2@I'ntrance of <br />the mine site, which shall be clearly visible from the access road, with the following information (Rule 3.1.12): <br />a. the name of the operator; <br />b. a statement that a reclamation permit for the operation has been issued by the Colorado~vlined Land <br />Reclamation Board; and, <br />~ c. the permit ntumber. <br />~~- 7. The boundaries of the permit boundary erne must be marked by momunents or other markers that are clearly <br />v _ visible and adequate to delineate such boundaries prior to site disturbance; <br />!~ ~ 8. It is a provision of this permit that the operations will be condutxed in accordance with the terms and <br />!! conditions iis6ed in yew application, as well as with the provisions of the Act and the Muteml Rules and <br />Regulations is effect at the time the permit is issued. <br />f`JL 9. Annually, on the anniversary date of permit issttartce, You must submit an annual fee as specified by Statute, <br />and ao annual report which includes a map describing the acreage affected and the acreage reclaimed to date (if <br />there aro changes from the previous year), any monitoring required by the Reclamation Plan to be submitted <br />annually on the amirersary date of the permit approval. Annual fees aro for the previous year a permit is held. For <br />example, a permit with the anniversary date of July 1,1995, the amual fie is for the period of July L, !994 dtrough <br />June 30, 1995. Failure to submit your annual fee and rcptxt by the ptxmit atutivetsary date tnay result in a civil <br />penalty, revocation of your permit, and forfeiture of your financial wazranty. The Mine Operator shall <br />notify the Division of Mitterais std Geology (DMG) of a reportable spill within the mined land permit arm using <br />the same timeframe regained by the permit, license. notice, slants, ruk or regulation governing the neportiog ofthe <br />spill to the other appropriate agency. Notice ofa reportable spill shall be FAXed to: Minerals Program Supervisor, <br />Division of Mittetala std Geology, FAX (303) 832-8106.
