Phase IVb Weekly Summary No. 7 Page 3 003-2203 <br />areas was observed. Golder observed Structural Fill (SF) placement in Phase IV for materials, lift <br />thickness, moisture content, and compactive effort. <br />Golder observed and documented excavation remediation activities associated with the Phase IV <br />underground working construction. Workings were designated as "Shallow or Deep" as defined in <br />the drawings and will be remediated accordingly. <br />Golder observed placement of SF, CSB, geogrid, CP and CR for undergound working remediation. <br />These materials were placed in accordance with the drawings and specifications into excavated or open <br />underground workings. Golder obtained samples of LVSCF for laboratory index testing. Materials <br />supplied met specifications. Ames conducted field testing and sampling of CSB and CP concrete as per <br />specifications, and will supply 28-day strength results. <br />Golder observed backfill of the geomembrane anchor trench for proper dimensions, lift thickness, <br />compaction, and use of suitable material. <br />Golder observed geomembrane repairs, documenting ambient air temperature, extrusion seaming, <br />and seam testing (trial seams, and vacuum). Geomembrane repaired in Phase IVb PSSA sump azea <br />met specifications. <br />Golder observed placement of LVSCF full time for correct material, road thickness, placement <br />thickness, drop height, and liner damage. LVSCF was placed in accordance with the specifications. <br />Golder observed placement of Low Volume Solution Collection piping for proper materials, <br />connections, and location. <br />Golder observed and documented confirmation drilling activities associated with the Phase IV <br />construction. Full-time monitoring was given to confirmation drilling of underground workings, <br />including logging holes, marking drill locations, surveying holes, and documenting underground <br />workings encountered. <br />Laboratory Tests <br />Golder obtained two samples of LVSCF (LVSCFb-03, and 04) for laboratory testing. The samples <br />tested met the requirements of the specifications and results are presented in Attachment B. <br />Meetines <br />CC&V held a Weekly Contractors meetings on December 3'a, l0a', and 17s'. Items discussed <br />included Safety, Environmental, Schedule, and CQA. The next weekly meeting will be held on <br />January 8, 2003 at 10:00 AM. <br />Golder Personnel On-Site <br />A summary of all Golder CQA personnel on site during this reporting period is presented in <br />Attachment C. <br />m~nm ~Dao~-s,.a~ Golder Associates Inc. <br />
