This amendment application is submitted in response to the Division's request that TR-04 be withdrawn and re-submitted as an amendment. As the basis for requiring the amendment was a change proposed for reclamation of one area of the operation, I believe that you indicated that we should submit with the application form only those exhibits which have changed since the previous amendment, together with a list indicating which exhibits have not changed from the previous amendment. Therefore, included with this application is an original and four (4) copies of each of the following: Regular 112 Operation Reclamation Application Form Amendment Application AM-002 Exhibits (list) Exhibit E -Reclamation (narrative) Exhibit F -Reclamation Plan Map Exhibit L -Reclamation Costs Exhibit Q - Proof Of Mailing Of Notices To County Commissioners And Soil Conservation District Exhibit R -Proof Of Filing With County Clerk Or Recorder Addendum I - Notice Requirements Certification Filing Fee - Check No. 194600 in the amount $1,750.00 The filing fee check submitted herewith has been reduced by $188.00, as we have been advised that the fee previously submitted with our technical revision request (TR-04) would be applied to the amendment application fee. Enclosed is a copy of the Division's March 4, 2002 letter verifying this. We appreciate your assistance in this process. Upon notification that the within application is accepted by the Division as complete, we will proceed with the Public Notice requirements.
