The site is <br /> within the drainage basin of Boxelder Creek but will not have any significant impact on that creek <br /> based on compliance with storm water management permit plans. <br /> The operator has acquired the applicable National Pollution Discharge Elimination System (NPDES) <br /> permits for this site from the Colorado Department of Health. See Exhibit N. <br /> 1. Sources of Ground Water on site or potentially affected by operations on site (see Map, <br /> Exhibit C): (List wells, springs, aquifers) None <br /> 2. Sources of Surface Water on site or potentially affected by operations on site (see Map, <br /> Exhibit C): (List tributary water courses, stockwater ponds, reservoirs, ditches, etc.) Unnamed <br /> tributaries of Boxelder Creek. No ponds, ditches or reservoirs will be affected. <br /> 3. Changes to drainage basins caused by operations on site (See Mining Plan and Reclamation <br /> Plan Map, Exhibits D and F): Except for minor changes caused by settling basins designed to trap <br /> sediments and prevent excessive erosion, there will be no changes to drainage basins. <br /> 4. All known aquifers and watersheds in this area: No identified aquifer in this area. Boxelder <br /> Creek watershed is the only known in this area. <br /> 5. Project water requirement estimates: Except for small quantities of water for human <br /> consumption and sanitation used by personnel working on site, and water for dust control, there will <br /> be no water required on site. Estimated water consumption is expected to not exceed 500 gallons <br /> per day. Water will be brought to the site from off-site. <br /> 6. Mitigation actions: None required. <br /> 7.
