The requests aze being made by Mr. Fred Holley as successor <br />operator. Permits M-1977-293 and M-1978-222UG are both located in Montrose County <br />and are held by International Uranium (USA) Corporation as current permittee. Also <br />included is a check in the amount of $200.00 from Mr. Holley to cover the two <br />application fees. International Uranium (USA) Corporation is forwazding the application <br />packages to the Division of Minerals and Geology for Mr. Holley. , <br />Two completed application packages aze provided for each active Permit. However, on <br />one copy of the Performance Warranty for Permit M-1978-222UG, the Warranty was not <br />notarized. On one copy of the transfer application form for Permit M-1977-293, Mr. <br />Holley checked item I. 1., but neglected to initial and date this item.
