Dear Ms. Chiappone,
The following letter and enclosed materials have been prepared to address the questions posed in
your November 18, 20041etter concerning the Riverbend Operation, Permit No. M-2000-016,
112 Construction Materials Reclamation Permit Amendment Application Package.
Application Form -Item 10 -Universal Transverse Mercator (UTM): Thomas Leidich and
Eric Reckentine discussed the UTM in question with Erica Crosby. During the telephone
discussion, a correct UTM was agreed upon and Erica noted the new number on the application
form.
Exhibit N -Source of Legal Right to Enter:
1) Phase 2: The page titled Affidavit -Phases 2, 3, 7, 8, located at the end of Exhibit N, was
signed by an officer of Mineral Reserves, Inc., allowing Lafarge West, Inc. the legal right
to enter the property for the purposes of mining and reclamation and the authority to
apply for all zoning and permits required for such operations.
2) Phase 5: The enclosed document titled Affidavit - Phase S was signed by an officer of
Mineral Reserves, Inc., allowing Lafarge West, Inc. the legal right to enter the property
for the purposed of mining and reclamation and the authority to apply for all zoning and
permits required for such operations.
3) Phase 7: The page titled Affidavit -Phases 2, 3, 7, 8, located at the end of Exhibit N, was
signed by an officer of Mineral Reserves, Inc., allowing Lafarge West, Inc. the legal right
to enter the property for the purposes of mining and reclamation and the authority to
apply for all zoning and permits required for such operations.
