Request for Corrections to Succession of Operators
On December 17, 2003, the Division of Minerals and Geology received an application form (Succession of
Operators Application) to transfer Pemut No. M-1984-001, which is permitted to J and M Schmalil, to Unaweep
Ventures LLC. The Division is returning puts of the transfer application form to you to have the Operator Name
(Permittee(Company Name) corrected on Pages 2, 4, and 5 to read "J and M Schmahl" and to have the person
that notarized your signature wmplete the "Notary for Permittee" section. The Division cannot transfer Permit
No. M-1984-001 to Unaweep Ventures LLC until all of the required documents are submitted and accepted.
Ms. Jessica Billingsley of Unaweep Ventures LLC called the Division this week and indicated that Unaweep
Ventures LLC would like the Reclamation Bond be recalculated aad that Unaweep Ventures LLC would like to
rename the site before the Division transfers the Permit. Therefore, when the Succession of Operators application
is completed by you the Division will then send Pages 1 and 2 to Unaweep Ventures LLC to have them make the
changes that they need to make, and after they return the application form to the Division then the Division will
begin the reclamation bond recalculation process. We request that you correct the form as soon as possible and
return it to my attention at the Division of Minerals and Geology, 1313 Sherman St., Rm 215, Denver, CO 80203.
