(b) Such map shall be based on site specific soils investigations and shall be on such a scale as to <br />provide a basis for soil management recommendations and be the same scale as the reclamation map; and <br />(c) provide, for each soil map unit, in tabular form, all data from analyses of representative <br />samples of surface and subsurface soil units as to: <br />(i) soil pH, texture, electrical conductivity, sodium adsorption ratio and any other <br />parameters that the Operator/Applicant or Office deems necessary for proper soils <br />characterization; <br />(ii} indicate on a map, or in the soils narrative the location of each soil unit on the <br />affected azea where the above soil chazacteristics maybe problematic as to <br />suitability for a plant growth medium; and <br />(iii) type, form and amounts of any soil amendments that may be necessary or <br />recommended by the local Soil Conservation Service, Soil Conservation District, <br />or other qualified special district, and standazd soil laboratory analyses and <br />fertilizer recommendations (if available) for the types of plant species proposed to <br />be established; or Page 152 Hard Rock/Metal Mining Rule 6 <br />(iv) provide, as an alternative to Subsubparagraph 6.4.20(17)(c), a plan of experiments <br />to determine the type, form and amount of any soil amendments that maybe <br />necessary to fulfill the requirements of the Reclamation Plan. <br />(18) Wildlife protection - In addition to the performance requirements of Section 3.1, the Office or Boazd <br />shall require the Operator to describe measures to minimize or prevent harm or damage to wildlife species and <br />habitat, including: <br />(a) adequately describe mitigation measures to ensure that there is no overall net loss of critical <br />or important wildlife habitat consistent with State of Colorado Division of Wildlife (DOW) and United States <br />Fish and Wildlife Service (LISFWS) recommendations, if any; and <br />(b) describe measures to prevent wildlife from coming into contact with designated chemicals, <br />toxic or acid-forming chemicals or azeas with acid mine drainage. <br />(19) Disposal of tailings and sludges in mine workings - In order to ensure the proper disposal of tailings <br />and sludges in mine workings, an Applicant shall comply with the provisions of Subsection 3.1.7. <br />ADDENDUM 1 -Notice Requirements -Affidavit of Postiue Notice (Bien) (Rule 1.6.2(11(b)l: Prior to filing <br />the application, post notices (signs) at the location of the proposed mine site, as required by the Office, of <br />sufficient size and number to cleazly identify the site as the location of a proposed mining operation giving name, <br />address, and phone number of the Applicant, and stating that (name of Applicant) has applied for a mining permit <br />with the Colorado Mined Land Reclamation Board. Anyone wishing to comment on the application may view <br />the application at the County Clerk's or Recorder's office and should send comments prior to the end of the public <br />comment period to the Colorado Mined Land Reclamation Office, at the address given on the cover of these <br />Rules and Regulations. Any class of Limited Impact 110 or 110d operation need only post notice (sign) at the <br />location of the proposed access to the site. After having posted such notice (sign), failure by an Applicant to <br />maintain such notice shall not constitute just cause to deny approval of the application. At the time the <br />application is filed with the Office, the Applicant shall provide a signed affidavit that such notice (sign) was <br />posted according to the provisions of this Rule. <br />Your application will not be considered submitted until the information listed above is received and found sufficient to <br />begin our review. A decision date will be established 90 days from the date of receipt of all of the requested informafion. <br />Additionally, if you have already published notice you will need to republish notice, after the Division considers the <br />application submitted. This notice must be published once a week for four (4) consecutive weeks, starting within ten (10) <br />
