Dear Mr. Davis:
On April 3, 1997 the Division of Minerals and Geology approved the Amendment application
submitted to the Division on December 2, 1996, addressing the following:
Add 116.5 acres to the permit azea, increase the affected land area by 85.5 acres, correct
the legal description, improve the reclamation plan.
With the following 3 commitments by Centennial Materials Inc., the terms of the Amendment
No. 3 approved by the Division aze hereby incorporated into Permit No. M-76-009-HR. All other
conditions and requirements of Permit No. M-76-009-IiR remain in full force and effect.
I. Detention sediment basin No. 1 is critical to all stages of both Phases 1 and 2.
Construction will be coordinated to be an eazly step in the initial stage of Phase
1. Construction of this detention sediment basin can only occur after the south
quarry wall is mined back faz enough and the crusher fines stockpile is moved to
the north side of the quarry floor. making room for the basin. Centennial
Materials will notify the Division when theses two items have been completed.
Construction of the basin will be completed within 120 days after the notification.
2. The final designs and specifications for the stormwater management facilities on
the quarry floor will be submitted to the Division at least 60 days prior to the
proposed date for start of construction of each facility.
3. The revegatation growth medium, will be a blend of waste rock, fines from the
crushing operation and salvaged soil. Testing of various blends of these materials
will be conducted to determine the best growth medium. A plan for testing to
determine the most suitable growth medium will be submitted to the Division
within 120 days of the approval date of this amendment.
