Box 1729 <br />Edwazds, CO 81632 <br />/ ./ <br />Re: Carol Ann Pit, Permit No. M-1997-026, Request for Succession of Operators Application, Name <br />Change Rider, and Performance W arranty <br />Dear Mr. Burkey: <br />On April 14, 2003, the Division of Minerals and Geology received an Increase Rider to Liberty Mutual Insurance <br />Company Corpofate Surety Bond No. 14-015-629, The Rider was submitted to the Division in the name of <br />"Oldcastle SW Group, Inc. DBA B&B Excavating". The Division, to date, has not received a Succession of <br />Operators Application ("SO" Application) form to transfer the Carol Ann Pit, Permit No. M-1997-026, from "B & <br />B Excavating, Inc." to "Oldczstle SW Group, Inc. DBA B&B Excavating". The Division will also need a <br />Performance Warranty executed in the name of "Oldcastle SW Group, Ino. DBA B&B Excavating" and a name <br />change Rider from Liberty Mutual Insurance Company to change the PerTnittee from "B & B Excavating, Inc." to <br />"Oldcastle SW Group, Inc. DBA B&B Excavating". We camlot accept the Bond Increase Rider u~il the SO <br />Application, the Performance Warranty, and the Name Change Rider have all been accepted.
