Specifically, the $750.00 civil penalty for NOV CV-2000-012 and the $1,000.00 civil <br />penalty for NOV CV-2000-01 1 are paid in full. We assume that you intend the payment of the <br />penalties to constitute final disposition of [he above-referenced Notices of Violation. <br />If, however, you still intend to file a request for an assessment conference or formal heazing for the <br />violations, then please do so in writing no later than June 22, 2001.
