RE: Herman Feit Pit, Permit No. M-1977-522, Application for Succession of Operators
Dear Mr. Lunbery:
On August 24, 2005, the Division of Minerals and Geology received an Application for Succession of Operators, a
Performance Warranty, a Financial Warranty/Check for Deposit in State Treasury and a $125 fee. The Division is
returning the Application for Succession of Operators and the Performance Warranty for the following:
Application for Succession of Operators:
The Division requires that the current permittee name be the same as the name on the permit (i.e. River Valley
Rock, Inc.) throughout this document.
The Division requires that the Permittee signature be witnessed by a Notary Public.
Performance Warranty:
The Division requires that the notary section be completed by a Notary Public.
Also, on the Application for Succession of Operators, Option A was initialed requiring the Division to recalculate the
bond prior to transferring the permit. Once the Application is corrected, the Division will start the process of
recalculating the bond.
