ripple Creek & Victor Gold Mining Company May 7, 2003 <br />Mr. Ron Roberts -2- 033-2203 <br />subgrade damage, until Polyflex returned to the site to perform certifiable repairs and tests on the <br />damaged geomembrane. The interim repairs completed by CC&V consisted of extrusion welds <br />(beads) of small punctures and scratches, and liner patches over the larger penetrations. Figure 2 <br />shows some of the interim patches on the liner. <br />On April 17, Golder inspected the panels and the interim repairs, and identified and observed the <br />following repair work: <br />• Covering of the interim extrusion weld beads with liner patches to provide additional <br />protection to the liner; and <br />• Application of a liner cap over the interim liner patches. The liner cap repair was selected to <br />minimize stress to the liner and provide coverage over the entire damaged area. Figure 3 <br />shows the liner cap placed over this area. <br />Documentation of the liner repairs for this area is detailed in Golder's defect log (Phase IVb PSSA <br />lower geomembrane) 80A through 80F. The repair work documentation will be presented in the <br />Phase IVb PSSA certification report. <br />ROCK DAMAGE REPAIRS <br />The geomembrane damage resulting from the rock incident primarily consisted of puncture <br />penetration to panels L426 and L436 (see Figure 4). Damage to panel L426, on [he PSSA slope, <br />consisted of four puncture penetrations through the liner. The punctures were approximately 6 to <br />10 inches in diameter In addition to puncturing the geomembrane liner, the underlying SLF surface <br />was damaged. Figure 5 shows a typical liner puncture on this panel. Figure 6 shows the typical <br />damage to the SLF in this area. <br />Damage to panel L436 consisted of ten puncture penetrations in the PSSA sump area. The puncture <br />sizes ranged from less than 0.25 to 6 inches in diameter. The underlying SLF had minor damage. <br />Figure 7 shows typical liner and SLF damage on this panel. <br />On April 17, Golder inspected the damaged panels, and identified and observed the following repair <br />work: <br />• Cutting out the damaged section of liner; <br />• Repairing the underlying SLF surface by filling the depression with SLF and hand <br />compacting and smoothing; <br />• Extrusion welding a liner patch over each damaged area; and <br />• For panel L436, extrusion welding a liner cap over the entire damaged area. <br />Figures 8 through 12 present photographs of the liner repair work on panels L426 and L436. <br />Documentation of the liner repairs for this area is detailed in Golder's defect log (Phase IVb PSSA <br />lower geomembrane) 79A through 790.
