PLEASE TAKE NOTICE that the Mined Land Keclamation Division ("the Division")
of Lhe Coloraao Department of )latural Resources has entered an order fixing
the amount of civil penalty ayainst you at 700.00 A copy of such
order is attacned 'Hereto.
You are required to pay such amount to the Division within Lhirty (30) days
after service cf tnis notice upon you.
If you wish to contest the amount of penalty or the fact of violation, you
must pay the amount of penalty to the Division within 30 days after receipt of
this notice ana request a public hearing before the i4ined Land Reclamation
board. Yeur request for a public nearing must be in writing and filed with
the Division within such 3U-day period. It should oriefly set forth the legal
and factual oasis of your oojection to the penalty or the findings or
violations.
Failure to pay such penalty amount and to request hearing within such period
of time will constitute a waiver by you of all further right to contest such
penalty or such violation.
