Cattany <br />Clifton, CO 81520 Acting Division Director <br />Re: Receipt of 110(c) to 112(c) Construction Materials Reclamation Permit Conversion Application, <br />Western Slope Flagstone Quarry No. 2, Permit No. M-1996-076, CN•1 <br />Deaz Mr. Fontanari: <br />On January 21, 2003, the Division of Minerals and Geology received your 110 to 112 Construction Materials <br />Reclamation Permit Conversion application for the Western Slope Flagstone Quarry No. 2, which is located in Mesa <br />County. All comment and review periods began on 7anuary 21, 2003. The decision date for your application is <br />scheduled For April 2l, 2003. <br />Within ten (10) days after submittal, you must publish notice in a newspaper of general circulation in the locality of <br />the proposed mining operation once a week for four (4) consecutive weeks. Immediately after the first publication, <br />mail a copy of the notice to all owners of record of surface and mineral rights, holders of any recorded easements, <br />and all owners of record of lands that aze within 200 feet of the boundary of the affected land. Proof of Notice and <br />mailings, such as Certified Mail - Retum Receipt Requested, must be submitted to the Division of Minerals and <br />Geology prior to the decision date.
