On August 11, 2006, the Division of Reclamation, Mining and Safety received a Transfer of Permit and Succession of
Operator Application, a Performance Warranty, a Financial Wamenty Certification of Deposit, and Certificate of
Deposit, in the amount of $10,667.00, for Benton Dawson Gravel, Permit No. M-1983-209. The Division is returning
the Transfer of Permit and Succession of Operator Application, Performance Warranty, and Financial Warranty
Certification of Deposit for the following:
Transfer of Permit and Succession of Operator Application:
The Division requires the Operator name be consistent throughout this document. The Successor Operator is
listed as both Frank Gilmore and Benson Dawson Gravel. Benton Dawson Gravel is also referred to as both
"Benton Dawson Gravel" and "Benson Dawson Gravel".
Performance Warranty:
The Division requires the Operator name be consistent throughout this document. The Notarization of
Cperator's Acknowledgement on page 3 lists Frank Gilmore as Owner/operator is Benson Dawson Gravel. If
Benson Dawson Gravel is the company name of the Operator, this must be noted elsewhere in this document.
If Frank Gilmore is the operator as an individual, Benson Dawson Gravel must be omitted from the
Notarization of Operator's Acknowledgement section.
Financial Warranty Certification of Deposit:
The Division requires the bank name and location be listed in the correct spaces and completed on page 2.
The Division requires the dollaz amount of the Certificate of Deposit be listed alphabetically on page 2.
The Division requires the completion of information pertaining to the initial date and expiration date of the
Certificate of Deposit be listed on page 2.
The Division requires the Operator name be consistent throughout this document. The Notarization of
Operator's Acknowledgement on page 4 lists Frank Gilmore as Owner/operator is Benson Dawson Gravel. If
Benson Dawson Gravel is the company name of the Operator, this must be noted elsewhere in this document.
If Frank Gilmore is the operator as an individual, Benson Dawson Gravel must be omitted from the
Notarization of Operator's Acknowledgement section.
