Roadside Mine (Permit No. C-81-041)
The referenced NOV was issued on May 1, 1996 by Barbara Pavlik,
based on an inspection of the Roadside Mine she conducted on March
28, 1996, and a water sample she collected on that date from CDPS
Discharge Point 004 (North Portal minewater discharge). I am
representing the Division in the conference because Barbara
resigned last month.
I have prepared the attached packet of relevant documentation.
Items included are as follows:
1. March 28, 1996 inspection report.
2. Water sample sheets and analysis report.
3. NOV C-96-10 and cover letter.
4. May 14, 1996 NOV abatement plan (Abatement Step 1).
5. May 21, 1996 Barbara Pavlik memo to assessment officer.
6. May 29 letter from Larry Reschke stating that abatement plan
had been completed by deadline.
7. June 3, 1996 Dan Mathews inspection report (for May 23
inspection), along with NOV termination notice and TSS analysis for
sample taken during the May 23 inspection.
8. Civil Penalty Notice and cover letter. Note that the Notice
was re-issued August 23, because the operator's request for
conference in response to the original Notice was apparently lost
in the mail.
