CN-1 / Natural Resource Trustee <br />Deaz Mr. Fontanari: <br />On April 22, 2003 the Division of Minerals and Geology approved the Conversion application submitted to the Division <br />on January 21, 2003, addressing the following: <br />Com~ersion of 110(cJ to 112(c) permit. <br />The teens of the Conversion No. 1 approved by the Division are hereby incorporated irno Permit No. M-1996-076. <br />The conditions of the Division approval are noted below. Please note that all other provisions of Permit No. M-1996-076 <br />remain in full force and effect. <br />Stipulation No. Description <br />1. Division is to be notified 60 days prior to any distnrbancz in North rnining area in writing. <br />Division of Mir~rals and Geology will recalculate bond at that tithe. No disturbance in North <br />area allowed u~il Division of Minerals and Geology acceptance of revised bond. <br />The estimated liability amount of $12,070.00 for the Westem Slope Flagstone Quarry No. 2, when added to the existing <br />liability amount of $3,630.00, exceeds the performance bond held. If you have not already done so, please submit <br />additional bond in the amount of $12,070.00.
