PLEASE TAKE NOTICE, that the Division of Mined Land Reclamation of the
State of Colorado Department of Natural Resources has determined, pursuant to
C.R.S. 1973, 34-33-123(7), that a pattern of violations exists as to the
requirements of the captioned permit, or as to the requirements of Article 33
of Title 34, Colorado Revised Statutes, at the captioned mine.
Furthermore, the Division has found that:
X Such violations are caused by the unwarranted failure of the Permittee
to comply with such requirements,
_ Such violations are willfully caused by the Permittee.

You are, entitled to a hearing before the Mined Land Reclamation Board
on this Order to Show Cause. Request for such shall be made within 30 days of
receipt of this Order to Show Cause why the permit should not be suspended or
revoked, and shall be held in accordance with Rule 5.03.5 of the Rules and
Regulations for Coal Mining.
