<br />Field Office (AFO) will be provided with a copy of the current <br />information. <br />BOND: The bond information was also not available at the <br />office where documents for the Meadows Mine are kept. DMG <br />again produced copies for review prior to the end of the <br />inspection. The bond is a corporate surety provided by Utica <br />Mutual Insurance Company. The current bond appears to be <br />adequate for the remaining liability on the site provided no <br />major obstacles are encountered. <br />NPDES OR DISCHARGE PERMIT: The Colorado Department of Health <br />has issued discharge permit COG-850012 for discharge of water <br />off the mine site. The permit was issued on 06/19/92 and <br />expires on 06/30/97. Records indicate that all discharges are <br />being monitored and are in compliance. With the exception of <br />monitoring of discharges no other surface or ground water <br />monitoring is being conducted at the mine site. The Colorado <br />program allows DMG to determine when monitoring is no longer <br />required and DMG has made that decision on the Meadows Mine. <br />However, since it will be several years before final release <br />of the area, the long term impact of mining on the hydrologic <br />balance will not be known with no monitoring being conducted. <br />POND P.ND IMPOUNDMENT INSPECTIONS: All pond and impoundments <br />are being inspected on a quarterly basis with the last <br />inspections being conducted on 04/20/94. Although the <br />inspections wr-_re being conducted as required they were not <br />being submitted to DMG by a registered profe::sional engineer. <br />DMG issued a Notice of Violation (NOV) addressing this non- <br />compliance. <br />CERTIFICATIONS: <br />Impoundments--All impoundments, with the exception of <br />the Northwest Sediment pond, were constructed with a <br />certified drawing showing the final configuration of <br />the structure. There was no certification for the <br />Northwest sediment pond. DMG issued a NOV addressing <br />this non-compliance. <br />SPECIAL CONDITIONS: The permit was issued with the stipulation <br />that the stability of the backfilled area be monitored and a <br />report of the monitoring be submitted on an annual basis. The <br />company was complying with this requirement even though the <br />report did not go into any detail. <br />The permit application contains a rill and gully plan which <br />requires the company to monitor erosion on the site on a <br />monthly basis. The company was not conducting the monthly <br />monitoring and DMG issued a NOV for this non-compliance. <br />4 <br />
