The <br />Division is returning these document for the following: <br />Succession Of Operator Application: <br />The Division requires receipt of a Financial Warranty with the submittal of Transfer of Permit and Succession <br />of Operator Application when Option B has been selected. <br />The Notary for Successor on page 5 needs correction. The Successor Operator's title and company name <br />should be listed. <br />Performance Warranty: <br />The Notarization of Operator's Aclmowledgement section on page 3 needs correcting.
