An assessment conference was held at the <br />Division's offir_e on Septmeber 8, 1994. In attendance at the <br />conference, in addition to yourself and me, were Susan eergmaier <br />and Larry Routten representing the Division and Brian Gontarek <br />and Terry Crowner representing P&M. If you agree with the <br />penalty, please sign the settlement agreement within ten (10) <br />days as instructed on the form. If you disagree with the <br />settlement agreement, do not sign it. The Division will fix the <br />penalty pursuant to Rule 5.04.3(5) and you may request a Board <br />Hearing.
