The Division is returning the Application for the following: <br />The Division requires that one of the options on page one of the application be initialed. <br />The Division requires some clarification of the operation name. Please indicate whether or not you would like <br />the operation name changed to Yuma County or remain as is. <br />In addition, the Division requires a $125 fee for this type of revision.
