We have determined that the following information must be submitted to <br />complete the requirements of Rule 4.17 of the Construction Material Rules and Regulations and C.RS. 34-32.5-117(5) <br />of the Colorado Land Reclamation Act for the Extraction of Construction Materials (a copy of this Rule and Statute are <br />enclosed for your information): <br />4.17.1(2) The Operator shall include in the notice to the Office the names and addresses and phone numbers of <br />all owners of record to the affected land. <br />4.17.1(3) The written notice requesting release shall be sent by Certi&ed Mail and be separate from other types <br />of communication to the Office. <br />If you have any questions, please do not hesitate to call. <br />Sincerely, <br />~i Susan M.
