Cattany <br />Steamboat Springs, CO 80477 ~ Acting Division Director <br />Re: Frmk & Hooker Pit, Permit No. M-1979-058/Receipt of 110 to 112 Construction Materials Reclamation <br />Permit Conversion Application Package, CN-1 <br />Dear Mr. Draper: <br />On July 5, 2002, the Division of Minerals and Geology received your 110 to 112 Constmction Materials Reclamation Pemtit <br />Conversion Application package for [he Funk & Hooker Pit, Permit No. M-1979-058. Preliminary review of the information <br />received detemriaed that the following items must be received before the Division can consider your application 'as being <br />submitted and technical review can begin: <br />APPLICATION FEE: The Division did not receive the entire $2,344.00 application fee with the application <br />package. <br />APPLICATION FORM: The Division requires use of an original application form. The Division requires that the <br />Operation name be the same as the name on the permit (i.e. Funk & Hooker Pit). Please be sure tbst all The <br />Responsibilities as a Permittee (pages 5 and 6) and the Signature page (page 7) aze completed. A decision date will be established 90 days from the date of receipt of all of the requested information. <br />Additionally, if you have already published notice you will need to republish notice, after the Division considers the application <br />submitted. This notice must be published once a week for four (4) consecutive weeks, starting within ten (10) days of the date <br />your application is considered submitted. We will notify you when you should initiate republication of your notice. The final <br />date for receiving comments is the 20th day after [he fourth publication or the next regulaz business day. <br />This letter shall not be construed to mean that there aze no other technical deficiencies in your application.
