This response addresses violation 1 of .4 <br />only. AFO has reviewed this response and finds tt to be appropriate. <br />The operator is being required to revise the permit to include <br />underdrain filter designs. The revision requirement is within the <br />30-day time limit established by OSM Directive INE-35. <br />The status of TDL 91-02-370-003 (TV4) is outlined below. AFO has <br />reviewed the record on the self-insurance issue as requested by letter <br />dated April 10, 1991, and your telephone request on April 28, 1991. The <br />record was reviewed to see if there 1s a contradiction ar inconsistency <br />of positions taken by AFO regarding the acceptability of self-insurance <br />under the approved Colorado State program. The results of that review <br />are included under 4 of 4 below. <br />The status of TDL 91-02-370-003 (TV4) is as follows: <br />1 of 4 - MLRD response has been found appropriate as of this date.
