As of the date of [his letter the Division has not received your annual fee and report for <br />January .ZO, 1995. Therefore, you are in potential violation of the above cited statute and the provisions of <br />your Permit No. M-89-084. <br /> <br />If you fail to submit your annual fee and report to our office by October 18, 1995, this matter will be <br />heard at a Formal Public Hearing before the Mined Land Reclamation Board for consideration of permit <br />revocation at the October 19, 1995 Board Meeting.
