Preliminary <br />review of the information received determined that tSxn (2) copies of the following items must be received before the <br />Division can consider your application as being submitted and technical review can begin: <br />APPT.TCATTnN 17nRM~ The application form received cannot be accepted any longer. The Mined Land <br />Reclaration Board approved new application forms on July 28, 2004. Please complete the enclosed form and <br />place a copy of the completed form at the County Clerk and Recorder's Office (along with a copy of all of the <br />requested E7dribits) and send the Original and a Copy of the form plus a receipt of filing with the County Clerk <br />and Recorder to the Division in Denver.
